I have read and agree to abide by the Penn Area Library Internet Policy, Guidelines and Procedures. I understand that any failure to adhere to these regulations will result in the loss of privileges.

I understand that Penn Area Library cannot be held responsible in any way for what I may or may not find on the Internet. I assume responsibility for sites I select to view on the Internet.

I understand that I am liable for any costs incurred for damage to hardware or software caused by my misuse.

If you are under eighteen (18) years of age and wish to use the Internet, the back of this card must also be signed by your parent or guardian in the presence of library staff.

As a parent or guardian, I have read the Penn Area Library Internet Policy, Guidelines and Procedures. I recognize that the Internet is an unregulated medium. I understand that it is impossible for the library to restrict access to controversial materials and I will not hold the library responsible for materials viewed or acquired via the Internet.
